Informed Consent Notice

The purpose of this research study is to understand how emotions are processed in humans.  It will take approximately 30-45 minute to complete this survey.  In this study, we will ask you to answer a number of questions about ratings of emotional responses and decisions you make in response to a number of questions asking you to make choices.  Answering the questions in the survey involves no foreseeable risks.  Participation is voluntary and you may stop at any time without penalty.  By completing the survey, you are giving consent to participate and confirming that you are at least 18 years old.   Results of the survey will be reported only on a group basis.

This survey is anonymous.
The record kept of your survey responses does not contain any identifying information about you unless a specific question in the survey has asked for this. If you have responded to a survey that used an identifying token to allow you to access the survey, you can rest assured that the identifying token is not kept with your responses. It is managed in a separate database, and will only be updated to indicate that you have (or haven't) completed this survey. There is no way of matching identification tokens with survey responses in this survey.
